Consumer Advocate Referral Form - UNY



IMPORTANT! If there is a life-threatening emergency,  please call 911.  Never refer life-threatening emergencies to an advocate.

IMPORTANT! Please do not send a referral to the advocate for these customer issues:
  • Commercial and Multi-Family accounts
  • Collection issues that do not include extenuating or unusual circumstance
  • Applicants under AI procedure (UCB, Pos ID and Residency)
  • Switched/Shared/Mixed metering 
  • High bill complaints/billing errors
  • Rate change
  • Disputes 
Customer Information
Please leave blank is there is no email address.
Verify customer understands they will receive a call back from an advocate and that the caller ID may or may not show a National Grid. Advise customer to make sure there is room in their voicemail box.
Referral Questions:

*Is this referral is outside of normal business hours? If so, please contact a supervisor if the customer is without power, uses life sustaining equipment, has a medical emergency, or has any other immediate threat to their health or safety. 

* If yes, please complete a financial statement prior to referring.


* Must complete a Financial Statement and provide options prior to submitting a referral,  Financial Statement SOP.
