Acceptable forms of evidence for a successful application include ONE of the following; proof of payment or receipt of equipment purchase, invoice or receipt from contractor installation, Massachusetts Clean Energy Center (Mass CEC) approved application, Mass Save rebate payment confirmation email or letter, or a photo of your heat pump’s outdoor unit showing the model and serial number. If providing model and serial numbers as evidence, please ensure both are legible within provided photo(s) for verification purposes.

I certify that all information provided in this application is accurate. The evidence submitted confirms that the installed equipment is a heat pump used to heat my home, either fully or partially. To qualify for benefits from the Heat Pump Rate program, I also certify that the National Grid residential account listed above is in my name, and the address provided is where the equipment is located and where the rate will be applied.
